Guardian Application - For Flights originating from Louisville
Please complete the digital application below, or click here for a printed version (PDF) that can be mailed. This application is to request consideration to serve as an Honor Flight guardian. Our staff will contact you once you are assigned to a veteran. If you have already submitted an online application but need to provide updated information, please go to the contact form at this link and include details in the Message field. Please do not resubmit this form.
Important notes:
- As a guardian, you must agree to donate the sum of $600.00 as determined by the Honor Flight Bluegrass Chapter Board of Directors to offset my costs (airfare, meals, etc.) involved with my participation in an Honor Flight mission. This fee is due by one week prior to your assigned flight day. As a 501(c)(3) charity, as determined by the IRS, your donation is tax deductible to the full extent of the law and shall be acknowledged by the Honor Flight Bluegrass Chapter.
- Guardians must be between the ages of 18-70, in good health and approved by the Honor Flight Bluegrass medical director. Spouses, domestic partners, or significant others of veterans are not eligible to serve as their assigned guardians.
- Once you are assigned to a mission, you must take a mandatory guardian training class to understand the role and responsibilities. In-person and online training options are available; however, we strongly encourage in-person attendance.
Thank you for supporting our veterans!
